The Care Transitions Learning Community is an informative and inspiring series with the goal of providing health care leaders with a learning experience that will help them make informed decisions about approaches to improve care transitions.
Over the course of the Care Transitions Learning Community participants will have the opportunity to participate in educational events each month.
The educational events will focus on specific components of the Affordable Care Act, opportunities released by the Center for Medicare and Medicaid Innovation, or best practice models for reducing re-hospitalizations and/or improving care transitions.
