To understand how we can better serve Central New York’s frail elders and children living in communities of poverty, the Community Health Foundation of Western & Central New York (CHFWCNY) conducted a survey among the region’s providers of acute, long-term, ambulatory and community-based care to identify the region’s health care gaps and service delivery issues. The analysis of the survey which was completed in April 2009 is attached to assist with and inform your work. Regional stakeholders are already using this document in their strategic planning efforts.
With efforts underway in New York State to address unmet community health needs, those involved with developing county-level Prevention Agendas toward the Healthiest State (Prevention Agenda) health assessments will find this information valuable and timely.
