ACCORD Corporation , Allegany County, NY: $30,000 to partner with Dr. Henry Chapman and participate in The Right Start Collaborative to Improve the Social, Emotional & Behavioral Well-being of Children Ages Birth to 5. This project will enhance the existing Head Start and Early Head Start program to foster resiliency by providing standardized social, emotional and behavioral health assessments and training.
Aftercare Nursing Services, Inc., Erie County, NY: $25,000 to lead a team participating in the Falls Prevention Collaborative, a fourteen month process to implement projects for community engagement and professional practice change to reduce falls and falls with injury. This team will focus on the community working with older adults in home care and in short-term rehabilitation. They'll look at assessment, education, and a study of the impact of exercise and balance programs.
Allegany Senior Foundation for the Allegany County Community Partnership on Aging, Belmont, NY: $70,000 to support a two-year county-wide effort to improve health-care decision-making for frail elders. The project, known as Sharing Your Wishes has been extended for one year with an additional grant of $35,000. In May 2008 an additional $15,000 was awarded to fund measurement of this project through 2010.
Allegany/Western Steuben Rural Health Network, Inc. , Schuyler & Steuben Counties, NY: $51,000 from the Southern Finger Lake Fund for inclusion of this network in the Sharing Your Wishes (SYW) coalition to provide SYW to Schuyler and Steuben Counties.
Allegany/Western Steuben Rural Health Network, Inc. , NY: $12,000 from the Southern Finger Lake Fund to provide support for the planning and implementation of a Strategic Business Plan with the goal of sustaining the Network's Health Literacy Initiative.
Alzheimer's Association WNY Chapter , WNY: $30,000 to serve as the team leader for a Quality Team participating in the Transitions of Care Collaborative. This team will work to improve transitions by working with caregivers to prepare them for future transitions of their family member.
American Federation for Aging Research Western & Central, NY: $25,000 to fund up to six scholarships for medical students in Western & Central New York into a program sponsored by several national organizations which focuses on training in geriatric research topics. The objective of this project is to interest current and future medical students in aging issues with the goal of incenting them to pursue academic medical careers in aging.
Baker Victory Services , Erie County, NY: $21,000 to use in collaboration with Catholic Charities to present a workshop and "Train the Trainer" session with national experts Dr. Robert Anda & Dr. Sandra Bloom on the "Impact of violence on Children's Health" throughout their lives. This grant is part of the Fellows Action Network.
Bassett Healthcare as fiscal agent for the NYS School Based Health Center Coalition: $19,900 to complete a report to inform schools in the 16 counties served by CHFWCNY on the school-based health care model including expert recommendations for dissemination of the information.
Bassett Healthcare (for NYS Coalition for School-Based Health Centers , Central New York: $86,435 to implement a strategy that will educate and inform CNY school districts with high percentages of children in poverty about the SBHC model. Interested districts are then eligible for additional resources to determine the viability of this model in their district.
Bethel Head Start, Inc. , Buffalo, NY: $30,000 to partner with the Mental Health Association and participate in The Right Start Collaborative to Improve the Social, Emotional & Behavioral Well-being of Children Ages Birth to 5. This project will address the specific mental health needs of children and their families who experience trauma as the result of exposure to violence.
Buffalo Hearing and Speech Center , Buffalo, NY: $30,000 to partner with the Northwest Buffalo Community Health Center and participate in The Right Start Collaborative to Improve the Social, Emotional & Behavioral Well-being of Children Ages Birth to 5. This project will increase meaningful communication between existing early intervention community systems, healthcare providers and families.
Buffalo Pediatric Society, Buffalo, NY: $7,500 to support the Buffalo Pediatric Society in their Vaccine Safety Awareness Radio Campaign.
California Institute for Health Systems Performance , Western & Central NY: $8,000 to assist in developing a white paper intended to help CHFWCNY understand and consider options for expanding the use of a quality improvement collaborative (QIC) approach that will assist in achieving the goals for the Foundation's target populations.
Catholic Charities of Buffalo Buffalo, NY: $150,000 to implement software and hardware system to streamline billing, records, and appointment scheduling. An additional $16,264 was awarded to support staff training and implementation of Lean Six Sigma across Catholic Charities.
Catholic Charities of Buffalo , Buffalo, NY: $139,000 to design a sustainable program which provides the needed "wrap-around" services to ensure the health, well-being and academic success of children living in communities of poverty who attend urban Catholic schools in Buffalo.
Catholic Health System Continuing Care Foundation , Erie County, NY: $25,000 to lead a team participating in the Falls Prevention Collaborative, a fourteen month process to implement projects for community engagement and professional practice change to reduce falls and falls with injury. This team will focus on the community working with older adults in at-risk communities of West Buffalo who participate in community centers.
Cattaraugus County Department of Aging , Olean, NY:$6,000 to expand the availability of Powerful Tools for Caregivers to benefit caregivers in Cattaraugus County. The funds will cover the training of class leaders and replacement leaders over a two-year period.
Center for Hospice & Palliative Care for the Coalition for Health Care Decision-Making, Cheektowaga, NY: $86,500 to support a two year county-wide effort to improve health-care decision-making for frail elders. The project, known as Sharing Your Wishes has been extended for one year with an additional grant of $28,641.13. In May 2008 an additional $15,000 was awarded to fund measurement of this project through 2010.
Central New York Regional Market Authority , Syracuse, NY: $21,000 to support the second year of operation for the Syracuse Mobile Market's Farm Fresh Mobile Market that provides fresh produce using a mobile market van service to neighborhoods in the City of Syracuse where no supermarkets exist.
Chapin Hall at the University of Chicago , Central New York: $30,000 to develop an environmental scan leading to an actionable strategy for improving birth outcomes for children in poverty birth to one year of age in Central New York.
Chautauqua Adult Day Care Centers, Inc., Chautauqua, NY: $5,000 in grant-writing assistance to increase the likelihood that Chautauqua County will be successful in their submission to the Office of Rural Health Policy Rural Health Network Development Planning Grant Program.
Chautauqua County Health Network , Jamestown, NY: $30,000 to support completion of an application for a Federally Qualified Health Care Center (FQHC) to provide primary, mental health and dental health care to the uninsured in Chautauqua County.
Chautauqua County Health Network , Jamestown, NY: $87,000 to provide scholarships for rural primary care providers who have significant numbers of frail elders in their practices to attend the Harvard Medical School Annual Review of Geriatric Medicine in 2009, 2010 & 2011.

This project will implement the evidence based program, Second Step, which is designed to reduce violence by developing children’s emotional competence and social skills.

Columbia University New York, NY: $150,000 to evaluate the Community Health Foundation’s Nuts & Bolts initiative and provide technical assistance to the initiative’s sixteen grantees.
Community Catalyst, Inc. WNY: $10,000 to conduct key informant interviews and a visioning session to support the WNY Health Equity Workgroup's efforts to develop effective interventions that will reduce health disparities within WNY.
Community Concern of WNY, Inc. , WNY: $30,000 to serve as the team leader for a Quality Team participating in the Transitions of Care Collaborative. This team will work to improve transitions by working with caregivers to prepare them for future transitions of their family member.
Community Concern of WNY, Inc. , WNY: up to $1,400 for Community Concern’s care manager to serve as a transition coach mentor for some of the WNY teams participating in the Transitions of Care Quality Improvement Collaborative.
Community Health Care Association of NYS , Western New York: $9,350 to support the establishment of the Safety Net Association of Primary Care Affiliated Providers (SNAPCAP) as a division of the Community Health Care Association of New York State, specifically covering the cost of a facilitator.
Community Health Center of Buffalo, Inc. , Buffalo, NY: $10,000 to help facilitate the opening of a Federally Qualified Health Center satellite of the Community Health Center of Buffalo in Niagara Falls, NY.
Community Health Center of Buffalo, Inc. , Buffalo, NY: $10,000 to support the Community Health Center of Buffalo in better positioning itself to submit a successful proposal under the American Recovery & Reinvestment Act of 2009 Funding Opportunity: Facility Investment Program (FIP).
Cornell Cooperative Extension of Monroe County , Western & Central, NY: $174,829 to serve as administrative bookkeeping support for the CHFWCNY Health Leadership Fellows program. In December 2007, an additional $183,700 was awarded for continued support. An additional $42,000 was awarded to cover 2009 expenses.
Cornell Institute for Translational Research on Aging Ithaca, NY: $107,646 to provide an objective evaluation of the cost-effectiveness and feasibility of the Project Home pilot at Loretto in Syracuse, NY – a three year demonstration program designed to respond to the wishes of an aging population to receive care in home and community based settings.
Cortland Regional Medical Center, Inc. , Cortland, NY: $30,000 to serve as the team leader for a Quality Team participating in the Transitions of Care Collaborative. This team will work to improve transitions between the hospital and rehabilitation and between rehabilitation and home.
Council on Aging of Niagara County for the Niagara Caregivers Network, Niagara Falls, NY: $69,519 to support a two year county-wide effort to improve health-care decision-making for frail elders. The project, known as Sharing Your Wishes has been extended for one year with an additional grant of $34,262. In May 2008 an additional $15,000 was awarded to fund measurement of this project through 2010.
Council on Aging of Niagara County , Niagara County: $30,000 to serve as the team leader for a Quality Team participating in the Transitions of Care Collaborative. This team will work to improve transitions of care by working with caregivers to prepare them for future transitions of their family member.
Crouse Hospital Syracuse, NY: $41,550 for Crouse Hospital in collaboration with Aurora of CNY to develop a curriculum to be used with various levels of hospital staff to develop awareness and specific skills to use in accommodating patients with vision and/or hearing loss.
Crouse Hospital , CNY: $30,000 to serve as the team leader for a Quality Team participating in the Transitions of Care Collaborative. The team will work on improving transitions from the hospital to home care or informal care at home.
Crouse Hospital , CNY: up to $2,400 for the transition coach from Crouse to serve as a transition coach mentor for the CNY teams participating in the Transitions of Care Quality Improvement Collaborative.
D'Youville College , Buffalo, NY: $25,000 to lead a team participating in the Falls Prevention Collaborative, a fourteen month process to impliment projects for community engagement and professional practice change to reduce falls and falls with injury. This team will focus on community dwelling older adults in West Buffalo and professional training within the Catholic Charities of Buffalo programs.
D'Youville College , Buffalo, NY: $18,000 in scholarship support for two final year nurse practitioner students per year for the next three years to encourage emphasis in geriatric care through their clinical experience.

Daemen College , Buffalo, NY: $18,000 in scholarship support for two final year nurse practitioner students per year for the next three years to encourage emphasis in geriatric care through their clinical experience.
Dentaquest Institute , Onondaga County, NY: up to $38,750 to fund the second phase of the CNY Children’s Oral Health Initiative aimed at treating children in poverty in Onondaga County. This phase includes an environmental scan, determination of the potential for a private dental practice learning collaborative, and the development and implementation of oral health training for primary health care providers.
Dentaquest Institute , Western & Central New York: $5,030 for Dentaquest Institute's Safety Net Solutions team to assist and advise CHFWCNY in its efforts to ensure successful implementation of the "Taking It To The Kids: Bringing dental education, prevention and treatment to places young people already go" project.
Erie 1 BOCES West Seneca, NY: $200,000 for planning and implementation of the Centers for Disease Control's Coordinated School Health Model in the Buffalo Public Schools. An additional $190,617 was funded to support year two of this program. Year three goals were supported by a grant in the amount of $165,135. $97,000 was awarded to fund the year four goals of this project.
Every Person Influences Children (EPIC) , Buffalo, NY: $150,000 to pilot the Ready, Set, Parent program which is designed to increase knowledge, confidence, and skills in parents of newborns.
